The Optional Tech Hub

For some families, technology becomes an added layer of care. The Cubby Tech Hub offers gentle enhancements that soothe and monitor your child’s sleep environment:

• Circadian lighting for healthier sleep–wake rhythms.
• Soft, sensory sounds to calm overstimulation.
• A remote monitoring camera so you can check in from another room.
• Two-way communication to comfort your child with your voice, even from afar.

These tools help you stay connected to your child and confident in the belief that they are safe, seen, and settled.

Cubby Beds are classified as FDA Class I medical devices (HCPCS E1399) and approved for Medicaid or private insurance coverage. The Arvada Pharmacy team handles every step of the paperwork, authorization, and installation.

What the Doctor Must Do

To begin, your doctor needs to complete the following packet steps:

1. Prescription (Rx) for the Cubby Bed.
2. Letter of Medical Necessity (LMN) — explaining your child’s needs and why lower-cost alternatives won’t work.
3. Patient demographics (name, DOB, diagnosis, history, etc.).
4. Return the packet to us via:
   • Fax: 720-707-2400
   • Email: [email protected] (encrypted for security)

Once we receive the documentation, we will submit everything to Medicaid for prior authorization (PAR). The review process usually takes 2–3 weeks. After approval, we handle delivery and installation of the bed.

Tech Hub (Optional Add-On)

Many families also want the Cubby Tech Hub, which adds:

• Remote monitoring camera
• Circadian lighting
• Soothing sounds & sensory support

Here’s the process:

1. We first submit a PAR for the Cubby Bed only.
2. After approval, we submit a separate PAR for the Tech Hub.
3. Medicaid will deny the Tech Hub under the standard benefit (this is expected).
4. Once denied, we will email-introduce you to your local Family Support Services Program (FSSP). Parents—not providers—file the Tech Hub request under FSSP waiver funding. Your case manager will then guide you through approval and payment.
